What is the SIR Enumeration Form?
The SIR Enumeration Form is the second stage after the Mapping process.
Once the mapping stage is completed, the BLO visits homes and distributes this form for verification and record purposes.

- Future correction processes

Step 1 — Important Preparation Before Filling the Form

As soon as the BLO gives the form:
- Make at least 3–4 photocopies immediately.
- The BLO usually does not provide another original form if mistakes happen.
It is strongly recommended to:
- Fill one rough copy first

- Father’s Name

However, if another legal guardian is responsible for you (for example grandfather or another family guardian), then their details should be entered instead.
Mother’s Information
Mother’s details are also required:
- Mother’s Name
- EPIC Number
Ensure spellings match official records.
Spouse Information
If married:
- Husband fills wife’s details
- Wife fills husband’s details
If unmarried, widowed, or spouse has passed away:
- Leave this section blank.

- Right Side

- Your name was NOT present in the old voter list
- You used a parent/grandparent/relative during mapping

- Father
- Mother
- Grandfather
- Grandmother
The relationship written here must exactly match the information provided during the Mapping stage.

Assembly Constituency Details
You will also need to fill:
- AC Name (Assembly Constituency)
- AC Number
- Part Number
- Serial Number
These details must match the old 2002 records correctly.

- Belapur
- AC No. 054

Step 4 — Understanding the Document Categories
The required documents depend mainly on:
- Your age
- Your historical voter records
The form follows a category system.
Category A
This category is for people whose names already exist in:
- 2002 voter list
- 2024 voter list
In this case:
- Only copies of those voter records are usually required.
Category B (Born Before 1 July 1987)
If a person:
- Is not in the 2002 list
- But was born before July 1987
Then they must provide:
- Relative’s 2002 documents
PLUS - One approved supporting document
